Job summary

Canyon Colorado Home Care & Hospice in Denver, CO is seeking a part-time Registered Nurse to provide intermittent skilled nursing in patients' homes. You will follow physician orders and the plan of care under the Clinical Manager's supervision, conducting initial evaluations and ongoing care planning with the IDT.

Qualifications

• Graduate of an approved school of professional nursing and licensed in the state(s) in which practicing.
• One year nursing experience preferred.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit, determines immediate care and support needs, and develops the plan of care with IDT.
• Regularly reevaluates nursing needs and revises the care plan accordingly.
• Initiates the plan of care and necessary revisions.
• Furnishes services requiring specialized nursing skill.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares clinical notes and summaries of care conferences in a timely manner as per policy.
• Coordinates services with physicians, other care providers, patient, and caregivers as appropriate.
• Informs physician and personnel of changes in patient condition and needs.
• Counsels patient and family in nursing-related matters.
• Participates in in-service programs and supervises other nursing staff.
• Adheres to Agency policies and procedures and maintains confidentiality.
