Job summary

Canyon Home Care and Hospice in Denver, CO seeks a Registered Nurse to provide intermittent skilled nursing at patients' homes under physician orders and a care plan managed by the Clinical Manager.

The role emphasizes teamwork, mutual respect, and collaboration, with supervision of LPN/LVN and HHAs, plus coordination with the IDT to ensure timely assessments and accurate documentation.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• One year nursing experience, preferred.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and develops the plan of care with the IDT.
• Regularly reevaluates nursing needs and revises the care plan.
• Initiates preventive and rehabilitative nursing procedures.
• Prepares notes and summaries per policy.
• Coordinates services with physicians, therapists, and caregivers.
• Informs physicians of changes in patient condition.
• Supervises LPN/LVN and HHA/CNA as required.
• Assigns home health aides with written instructions.
• Completes OASIS assessments and discharge summaries.
• Teaches other nursing personnel and participates in QA activities.
• Maintains confidentiality and adheres to agency policies.
