Job summary

Optimal Hospice is seeking an Admin On-Call Registered Nurse to provide comprehensive hospice care for our patients in the Denver area. This 7-on/7-off position focuses on assessment, care planning, and ensuring comfort.

You will collaborate with the interdisciplinary team, educate families, and maintain thorough clinical notes. A valid RN license, CPR, and driver’s license are required, with hospice experience preferred.

Qualifications

RN license in good standing
Graduate of National League for Nursing accredited school of nursing
Maintains a current CPR certification
Valid driver’s license required
At least one year of hospice experience preferred

Responsibilities

Completes initial, comprehensive and ongoing patient assessments and family assessments.
Provides professional nursing care by utilizing the nursing process.
Develops and revises the care plan with input from the interdisciplinary team.
Administers medications and treatments as prescribed by the physician.
Acts as Case Manager for assigned caseload.
Maintains timely and accurate clinical notes and documents changes in condition.
Coordinates with physicians and community health professionals to plan care.
Educates patient and family on self-care and medication management.
Participates in interdisciplinary group meetings.
Ensures a safe environment and continuity of care for patients.
