Job summary

Delta Dental of Minnesota is seeking an Associate Care Coordinator to help Government Program members access dental services, schedule appointments, and overcome barriers to care. The role emphasizes clear communication with members, case managers, and dental offices.

You will make reminder calls, arrange transportation or interpreter services when needed, and follow up after visits to determine if additional care is required.

Qualifications

• High School Diploma or equivalent required; related experience preferred.
• Experience with Minnesota Health Care Programs preferred.
• Ability to maintain confidentiality and communicate clearly with diverse individuals.

Responsibilities

• Assist Government Program members in obtaining access to dental services.
• Arrange transportation and interpreter services when applicable.
• Coordinate with dental offices and make reminder calls.
• Follow up after appointments to confirm attendance and determine whether further follow-up is needed.
• Assist customer service based on forecast and call volume.
• Other related duties as assigned.
