Job summary

Harmony Senior Services is seeking a dedicated program leader to oversee Harmony Square activities for residents with dementia. You will supervise the Harmony Square team and work with the Healthcare Director to deliver engaging programs and events.

You will coordinate monthly planning, plan weekly outings, and collaborate with clinical staff to meet resident needs and ensure safety and quality of life. A valid driver’s license and willingness to work flexible hours are required.

Qualifications

• Three to five years’ experience working with individuals with dementia including leading programs and educating staff/families.
• Dementia training or Certified Dementia Practitioner preferred or eligible for certification.
• Strong organizational and time management skills.
• Experience supervising and managing staff.
• Must have a valid driver’s license and good driving record.

Responsibilities

• Supervise and direct Harmony Square team members in conjunction with the Healthcare Director.
• Develops, plans, and implements resident programs to enrich lives of residents and families.
• Plan and schedule weekly outings and lead 3-4 high quality programs per day.
• Assess resident needs in partnership with clinical team and update Healthcare Director and Executive Director.
• Participate in Resident/Family Care Conferences and Family Call Program.
• Develop relationships with residents and families to promote participation and satisfaction.
• Ensure dining aspects are met in partnership with Director of Dining and Healthcare Director.
• Complete state required care plans and service plans with Healthcare Director.
• Lead daily huddles and neighborhood rounds; attend monthly programming calls.
