Salary: $100,000 - $107,000 per year

Catholic Charities of Baltimore, Caritas House is seeking a Delegating Nurse Manager to provide nursing leadership and collaborates with an interdisciplinary team to support the medical health and well‑being of individuals residing in residential programs. This role provides nursing oversight and direction to direct care professionals in implementing the medical components of individual care plans and ensures care is delivered in accordance with established nursing standards, care plans, and applicable requirements. The Delegating Nurse also supervises the Part‑Time Delegating Nurse, providing guidance, support, and oversight in the delivery of delegated nursing services.

The work schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. This position requires flexibility to provide on‑call support as needed. As this is a 24‑hour facility, the employee may be contacted for emergencies outside of regular business hours, including evenings, nights, and weekends.

JOB DUTIES & RESPONSIBILITIES:
  • Provides nursing leadership for residential programs by guiding the medical components of individual care plans and promoting safe, person‑centered care for assigned residents.
  • Manages Delegating Nurse responsibilities and supervises the Part‑Time Delegating Nurse by providing clinical direction, coaching, performance feedback, and oversight to ensure delegated nursing services are completed in accordance with established standards and applicable requirements.
  • Oversees case management for assigned residents by assessing health needs, coordinating medical services, monitoring changes in condition, and ensuring timely follow‐up with providers and interdisciplinary team members.
  • Develops, revises, and monitors nursing diagnoses, care plans, goals, and interventions to address identified health needs and support positive resident outcomes.
  • Delegates nursing functions to licensed and unlicensed direct care professionals and monitors medication administration, delegated tasks, and staff competency for the assigned caseload.
  • Trains, supports, and advises direct care professionals and Certified Medication Technicians on care plan requirements, medical procedures, treatments, and nursing services to promote safe and consistent implementation.
  • Completes the required 4‑hour recertification course and provides training to CMTs to support timely renewal of CMT certification.
  • Coordinates with residents, families, providers, direct care professionals, and interdisciplinary team members to clarify care needs, connect resources, resolve service barriers, and support continuity of care.
  • Completes required nursing assessments, evaluations, reviews, reports, and health documentation accurately, consistently, and within established timeframes.
  • Reviews documentation, care delivery concerns, and data quality issues to identify trends, correct gaps, and support compliance with Agency expectations, nursing standards, and regulatory requirements.
  • Makes appropriate clinical decisions within scope of practice and promptly escalates significant health changes, service barriers, safety concerns, or compliance risks to the Director and appropriate stakeholders.
  • Provides or coordinates 24/7 on‑call nursing coverage to ensure timely clinical guidance, response to urgent health concerns, and continuity of care for residents.
  • Advises the Director on resident health needs, nursing services, staffing considerations, and opportunities to strengthen care coordination, compliance, and program performance.
  • Performs other duties as assigned.
EDUCATION & EXPERIENCE REQUIREMENTS:
  • Associate degree from an accredited nursing education.
  • Maintains Delegating Nurse certification in compliance with state regulations.
  • Three years of nursing experience, including at least one year of experience in long‑term care, dementia care, and/or acute care.
  • Working knowledge of Medicare skilled level criteria and rehabilitation.
Certification/Licensure:
  • Registered Nurse (RN) license in good standing with the Maryland Board of Nursing (MBON).
  • Current CPR and First Aid certification

  • Requires proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook. Knowledge of other Microsoft Office applications, such as Word, Excel PowerPoint, Teams, and OneDrive is preferred.
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
  • No remote or hybrid work availability based on core and essential functions of the position.
  • Flexibility may be required to work beyond standard hours, including weekends and holidays, as needed.
  • Adjusting or moving objects up to 30 in all directions, which may include lifting, carrying, pushing, pulling, or repositioning items as necessary to perform job duties.
  • Engaging in communication with others to exchange information, which may involve speaking, listening, writing, or using electronic communication methods to ensure clear and effective understanding. This includes both verbal and non‑verbal interactions with colleagues, clients, or external parties.
  • Repetitive motions involving the wrists, hands, and/or fingers, including tasks such as typing, data entry, or handling small objects. These repetitive activities may occur throughout the workday and require attention to ergonomics to ensure comfort and minimize strain.
  • Assessing the accuracy, neatness, and completeness of assigned work to ensure it meets quality standards and adheres to established guidelines.
  • Hearing and vision abilities, with or without correction, sufficient to drive safely and effectively observe and communicate with others.
