Job summary

The League For People with Disabilities in Baltimore, MD is seeking a DDA Case Manager to support adults with developmental and intellectual disabilities. Qualifications

• Bachelor's degree in a related field required.
• Minimum 2 years of case management or support coordination experience preferred.
• Knowledge of DDA regulations and person-centered planning.
• Familiarity with Medicaid waiver services and electronic documentation systems.

Responsibilities

• Coordinate and monitor PCPs with participants, families, and care teams.
• Advocate for participants' rights and inclusion in Day Habilitation and the community.
• Maintain accurate, timely, and compliant records per DDA, Medicaid, and agency standards.
• Prepare reports, progress notes, and assessments showing outcomes and impact.
• Collaborate with Day Hab staff, clinicians, and support professionals.
• Support audits and continuous improvement efforts.
