Job summary
Center for Human Development, Inc. is seeking a part-time Developmental Disabilities Eligibility Specialist for 30 hours/week to provide intake and entry into the DD program. You will establish eligibility, enroll individuals, and coordinate with families seeking services in Oregon.
The role requires a mix of interviews, data entry, and coordination of evaluations to determine eligibility and initiate the planning process with Service Coordinators.
Qualifications
- Bachelor's degree in behavioral or social science or closely related field.
- Bachelor's degree in any field and one year of human service-related experience.
- Associate’s degree in behavioral or social science or closely related field and two years of human services related experience
- Three years of human services related experience
Responsibilities
- Interview applicants and/or legal representatives to complete the application and documents.
- Enter new individuals into the case management system.
- Open/close eligibility in the State platform as needed.
- Review records and coordinate evaluations to determine eligibility.
