Graceworks Enhanced Living is seeking a dedicated supervisor to manage Direct Care and Home Supervisor roles, ensuring a safe, home-like environment for individuals served in alignment with the agency’s mission and regulations. The position emphasizes staffing, oversight, and program quality across IPs.

The role involves budget adherence, audits, staff development, and collaboration with clinical and support teams to promote self-direction and well-being for residents.

Qualifications

• Bachelor’s degree from an accredited program in Human Service or related field.
• Valid driver’s license with less than 6 points.
• At least two years of DD programmatic experience with two to three years supervisory experience preferred.
• QIDD Certification preferred.
• Current Medication Passing, CPR and first aid certification is a plus.
• Experience with dually-diagnosed persons or behavioral problems is a plus.

Responsibilities

• Manage the home in adherence to Federal, State, and local guidelines.
• Ensure rights and welfare of individuals served are guaranteed.
• Develop and monitor Plan of Correction with ICF director.
• Conduct 90 day and annual employee appraisals and run staff meetings.
• Recruit, orient, train, and monitor Direct Care and Home Supervisor staff.
• Oversee home supervisor to ensure equitable coverage based on staff and client needs.
• Provide 24 hour contact with the home via phone.
• Maintain budget for programming, staffing, groceries and supplies.
• Report and document MUI’s/UI’s timely; develop corrective action plans.
• Coordinate with consultants and volunteers; ensure quality programming and audits.
