Disability Services of the Southwest (DSSW) seeks a passionate DBMD Case Manager to support individuals with deaf-blindness and multiple disabilities in achieving independence and community inclusion in Austin, TX. This exempt, full-time role coordinates services, advocates for clients, and ensures program standards are met within the DBMD waiver.

Qualifications

• Bachelor's degree in health or human services with at least two years of direct disability services experience or equivalent.
• Associate degree in health or human services with at least four years of direct experience.
• High school diploma with at least six years of direct disability services experience.

Responsabilités

• Manage daily activities and operations of assigned DBMD program participants.
• Monitor service delivery and ensure quality outcomes for caseloads.
• Coordinate enrollment and onboarding of eligible applicants.
• Ensure compliance with DBMD program requirements and regulatory standards.
• Advocate for clients and safeguard individual rights.
• Collaborate with families, providers, healthcare professionals, and community partners.
• Participate in trainings, meetings, and ARD meetings as applicable.
• Provide crisis intervention and problem resolution when needed.
• Maintain accurate and timely documentation.
• Support on-call responsibilities and client care activities.
• Travel throughout the Austin service area for client visits and assessments.
• Maintain confidentiality and comply with policies.
