Job summary

Moments Hospice is seeking an experienced RN Case Manager to lead clinical coordination for patients and families in the Milwaukee area. You will manage symptoms, coordinate care, and provide compassionate guidance during end-of-life care.

The role supports a field-based schedule with a Monday–Friday workweek, offering a fleet car program, competitive benefits, and opportunities to influence patient outcomes and care team collaboration.

Qualifications

• Active RN license in state you work
• Proficiency with EMR systems
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently with high standards
• Compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care
