Job summary

The role is not remote and requires flexibility to a Sun-Thursday or Tuesday-Saturday schedule. Experience in hospice or hospital settings and proficiency with EMR/EHR systems are preferred.

Qualifications

• RN license required with multi-state option within 30 days.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• English communication required; Spanish preferred.
• Strong decision-making and problem-solving skills.
• Excellent clinical and assessment skills.

Responsibilities

• Triages phone calls and ensures callers’ needs are met with excellent customer service.
• Provides direct care to patients and families incorporating psychosocial, spiritual, cultural, physical and biological components.
• Handles clinical inquiries related to Chapters Health System’s services in a timely, courteous manner.
• Explains benefits of Chapters Health System’s services clearly to patients and families.
• Gathers necessary information and documentation for accurate patient records in the EMR.
• Enters and tracks patient referral data in the EMR and coordinates in-person visits as needed.
