Job summary

St. John’s Community Services is seeking a Day Support Specialist to provide direct support to individuals with disabilities in community settings.

The role includes assisting with daily routines, data collection, and documentation under the Individual Service Plan (ISP). You will participate in activities, coordinate transportation, and help maintain health and safety while collaborating with a multidisciplinary team.

Qualifications

Ability to gain working knowledge of SJCS policies and procedures.
Ability to act as a representative of SJCS to the public.
Ability to effectively communicate and coordinate with other staff members and individuals from diverse backgrounds.

Responsibilities

Carry out mission and core values through relationship-building and ISP-driven skills training.
Facilitate active participation in community activities and contracted services.
Complete daily data collection and documentation per ISP, including behavioral support.
Observe and report on individuals' needs and changes requiring follow-up.
Monitor and report unusual events per SJCS policy and state regulations.
Develop activity calendars to meet ISP goals.
Provide safe transportation to activities and appointments.
Supervise health and safety, including medication administration as required.
Attend agency events, trainings and meetings.
