Qualifications

Responsibilities

• Delivers safe and appropriate care to patients per nursing skills inventory and patient needs.
• Coordinates nursing care planning with interdisciplinary team.
• Collaborates with healthcare team to achieve positive outcomes using data, research, and standards of care.
• Coordinates treatment plans and develops discharge/transfer plans.
• Participates in program evaluation, quality improvement, and patient experience initiatives.
• Adheres to Memorial Hermann policies and regulatory standards; maintains time and supply management.
