Atlas Post Acute at Woodbury Country Club in Woodbury, New Jersey is seeking a dedicated Full Time DAY Shift Nurse to join our healthcare team. The role focuses on delivering high-quality, person-centered care to residents and coordinating with rehab and behavioral health teams to support recovery and independence.

You will assess, plan, implement, and evaluate individualized care plans while maintaining accurate records and upholding dignity and comfort for our residents.

Qualifications

• Current licensure as a Registered Nurse in New Jersey.

Responsabilités

• Deliver high-quality, person-centered care to residents.
• Conduct thorough initial and ongoing assessments, including physical, cognitive, and emotional assessments.
• Develop and implement individualized care plans that address residents' unique needs and goals.
• Collaborate with interdisciplinary team including rehab, behavioral health, and social services.
• Provide education and support to residents and families to promote health and well-being.
• Maintain accurate and timely records of patient care and services.
• Maintain a clean, safe environment that promotes patient comfort and dignity.
