We are seeking a compassionate and dedicated Full Time DAY Shift Nurse to join our team at Atlas Post Acute at Woodbury Country Club in Woodbury, New Jersey. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care to our residents.

About the Role:

As a Full Time DAY Shift Nurse, you will work closely with our interdisciplinary team to provide comprehensive care to our residents, including those with complex medical and behavioral needs. You will assess, plan, implement, and evaluate individualized care plans to ensure the highest standards of patient care and satisfaction. You will also collaborate with our rehabilitation team to design and implement individualized treatment plans that promote optimal recovery and independence.

Responsibilities:

• Deliver high-quality, person-centered care to our residents, incorporating evidence-based practice and clinical expertise
• Conduct thorough initial and ongoing assessments, including physical, cognitive, and emotional assessments
• Develop and implement individualized care plans that address the unique needs and goals of each resident
• Collaborate with our interdisciplinary team to provide comprehensive care, including rehabilitation, behavioral health, and social services
• Provide education and support to residents and their families to promote optimal health and well-being

Requirements:

• Current licensure as a Registered Nurse in New Jersey
