Responsibilities

• Assesses patient needs and discharge planning.
• Formulates and revises plan of care in collaboration with the health team.
• Implements care through direct patient care and supervision of staff.
• Evaluates patient progress and adjusts care as needed.
• Documents assessment, planning and evaluation in the patient record.
• Provides care based on evidence and participates in education and mentoring.
• Contributes to improving patient outcomes through high-quality nursing care.
• May supervise staff and workflow in the department.
• Interacts with multiple healthcare professionals to coordinate care.
