• BLS certification required, with ongoing certification maintenance.

Responsibilities

• Coordinate patient care and discharge planning.
• Assist in referrals and coordinate resources for patients; maintain documentation.
• Collaborate with physicians, nurses, social workers, and insurance providers to ensure seamless care planning.
• Maintain case management data and support analytics/reporting.
