Job summary

Jackson Creek Post Acute is seeking a licensed practical nurse/licensed vocational nurse (LPN/LVN) to provide direct nursing care to residents and supervise nursing assistants. Qualifications

Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved LPN/LVN program.
Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
Ability to read, write, speak, and understand English.

Responsibilities

Direct the day-to-day functions of the nursing staff and supervise nursing assistants.
Ensure compliance with facility policies, procedures, and regulatory guidelines.
Review and update nursing policies, procedures and discharge plans as required.
Perform and supervise nursing care in accordance with physicians' orders and residents' care plans.
Maintain documentation, reports and records as required by the facility.
Coordinate with other departments to ensure the resident's care is comprehensive and safe.
