Job summary

Sweetwater Care in Montana is seeking an LPN with leadership experience to oversee nursing care within facility guidelines. You will work under physician orders and center policies, ensuring patient needs are met through timely care and education.

The role requires current LPN licensure in the state, with preferred experience in long-term care and PointClickCare charting. This position is on-site and focuses on quality improvement and patient safety.

Qualifications

• Licensed LPN in state.
• 1 year nursing management experience preferred.
• Long-term care experience.
• Point Click Care charting experience.

Responsibilities

• Demonstrates knowledge and appropriate use of infection control policies.
• Participates in Quality Improvement Program.
• Utilizes Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely per facility policy.
• Administers treatments timely per facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Might make rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing managementexperience preferred
