Job summary
Trinity Health Livonia Hospital is seeking a dedicated Case Manager to coordinate patient care from admission through discharge, applying disease management and utilization review principles. The role supports a variety of settings including Emergency, Birthing Center, or medical/surgical floors, depending on patient needs.
The successful applicant will hold a Bachelor of Science in Nursing or health-related field, be licensed in Michigan, and pursue case management certification while
Qualifications
- Bachelor of Science degree in Nursing or health related field or equivalent work experience.
- Current licensure to practice in Nursing in the State of Michigan.
- Progression toward or a completed certification in case management.
- Current Basic Life Support (BLS) certification.
- Five years of clinical nursing experience with recent utilization review/management/discharge planning or case management.
Responsibilities
- Assess patient needs and plan care across admission to discharge.
- Collaborate with physicians and others to develop safe care plans.
- Coordinate inpatient planning using criteria sets.
- Work in EC, Birthing Center, or medical/surgical floors depending on assignment.
