Qualifications

• High School diploma or GED preferred.
• 6 months–1 year CNA experience preferred.

Responsibilities

• Perform basic nursing care under the supervision of a licensed nurse.
• Follow treatment plans to meet residents’ needs including meals, transfer, ambulation, bathing, toileting and grooming.
• Observe and communicate resident condition to facility staff.
• Take and record vital signs and weight as needed.
• Document care and observations in worksheets and EMR promptly.
• Maintain infection control practices and safe equipment use.
• Support residents’ psychosocial needs per care plan.
• Attend mandatory trainings and satisfy 12 hours/yr CEU requirements.
• Collaborate with care team and assist coworkers as needed.
• Follow facility policies and perform other duties.
