Job summary

Qualifications

• Education: High School diploma or GED preferred.
• Experience: 6 months – 1 year experience as a CNA preferred.
• State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.

Responsibilities

• Performs basic nursing care procedures under the direction and supervision of a licensed nurse.
• Observes and communicates pertinent information about residents' condition to staff.
• Takes and records pulse, respirations, blood pressures and weight as needed.
• Documents care and observations on work sheets and EMR accurately and timely.
• Maintains cooperative relationships to enhance the efficient operation of the care team.
• Adheres to infection control practices.
• Supports psychosocial needs of residents as per plan of care and profile.
• Attends mandatory education and 12 hours of continuing education per year.
