Beacon Health System's Memorial Hospital in South Bend, IN, is hiring a full-time day shift Clinical Pharmacist to provide accurate dispensing and patient-focused pharmaceutical care. You will collaborate with physicians and healthcare teams, ensure regulatory compliance, supervise staff, and participate in medication therapy management.

Qualifications

• Bachelor's degree in Pharmacy or PharmD with licensure in Indiana or eligible for licensure.
• Strong communication and teamwork skills.
• Experience in hospital pharmacy or clinical pharmacy a plus.

Responsabilités

• Interpret and transcribe physician orders; verify medication packages; prepare IV admixtures.
• Monitor therapy for interactions, allergies, dosing; assist with dosing services; report adverse events.
• Provide drug information and clarify therapy alternatives to staff, patients and families.
• Maintain formulary compliance and promote rational medication selection; minimize non-formulary procurement.
• Ensure proper handling, storage and safety of medications; supervise staff as needed.
• Maintain accurate records of procurement, dispensing and usage; ensure proper charging and inventory.
• Attend meetings, adhere to policies, and be available for overtime or additional shifts as needed.
