Job summary

Qualifications

Responsibilities

• Provide case management services to program residents, including intake, assessment, service planning, and ongoing support.
• Coordinate client intakes, including communication, documentation, and transportation arrangements.
• Complete required assessments, safety plans, treatment plans, and documentation within established timeframes and regulatory standards.
• Maintain accurate, organized client records and ensure compliance with documentation systems.
• Facilitate and participate in treatment team meetings, case reviews, and care coordination with external partners.
• Monitor client progress, update plans as required, and provide regular status reports to stakeholders.
• Support residents in achieving goals related to parenting, education, employment, housing, and independent living skills.
• Ensure residents' basic needs and living standards are maintained.
• Provide leadership, guidance, and oversight to staff on documentation and client care practices.
• Coordinate appointments, referrals, and community resources, including healthcare, childcare, and housing services.
• Participate in on-call rotation and program operations, including meetings, trainings, and quality improvement initiatives.
• Assist with program administration tasks such as billing support, fee tracking, and reporting.
• Conduct weekly individual meetings with residents and facilitate group programming.
