Job summary

Albertina Kerr Centers in Beaverton is seeking a Day Services Program Coordinator to organize ISP development, documentation, and transitions for individuals in Day Support Activities. You will coordinate ISPs, track progress, and ensure timely communication among families, providers, and staff.

The role requires a 4-year degree or 2+ years of case management, plus proficiency with Microsoft Office and electronic records software.

Qualifications

  • Bachelor's degree in a related field and at least 2 years of case management experience OR 4 years of relevant experience with foundational knowledge of social services.
  • Proficiency with software applications and tracking systems, including Microsoft Office (Excel, Teams, Outlook). Electronic Records software experience preferred.
  • Experience hosting and participating in virtual meetings and training modules.
  • Strong problem-solving skills with the ability to manage conflicts and de-escalate challenging behaviors or situations.
  • Excellent time management and organizational skills with strong attention to detail.
  • Adaptability to changing priorities and environments.
  • Effective and empathetic communication skills; comfortable engaging with clients and presenting to groups of varying sizes.
  • Ability to meet all background check, reference, and driving requirements.

Responsibilities

  • Coordinate ISP development, updates, packets, meetings, training, documentation, and follow-up for assigned people supported.
  • Monitor progress toward ISP goals and partner with CSS staff, Program Managers, families, home providers, case managers, SC/PAs, and other team members to address changing support needs.
  • Review referral packets, coordinate intake paperwork, track service authorizations, and support exits or transitions with clear communication and timely documentation.
  • Coordinate updates related to medical, behavioral, safety, attendance, adaptive equipment, or other person-specific supports, including staff training documentation when needed.
  • Communicate professionally and empathetically with people supported, families, advocates, internal teams, and external partners to support dignity, safety, inclusion, and continuity of services.
  • Provide direct "on the floor" support as needed, including assistance with ADLs and IADLs, and prepare clear, timely reports and related documentation.

We are seeking a detail-oriented, compassionate Day Services Program Coordinator who enjoys organized case coordination, collaborative problem-solving, and advocacy-focused work. In this role, you will support individuals participating in Day Support Activities (DSA) by coordinating Individual Support Plans (ISPs), maintaining accurate documentation, supporting intake and transition processes, and serving as a key communication link among people supported, families, home providers, case managers, Services Coordinators/Personal Agents (SC/PAs), Program Managers, Community Support Specialists, and other support team members.
