Job summary

Albertina Kerr seeks a Day Services Program Coordinator to coordinate ISP development, updates, and documentation for individuals receiving day support services. You will work with families, home providers, case managers, and teams to address changing support needs and ensure timely communications.

The role requires a Bachelor's degree and 2+ years of case management or 4+ years of relevant experience, plus proficiency with Microsoft Office and electronic records software.

Qualifications

  • 2+ years of case management experience or 4+ years of relevant experience.
  • Proficiency with Microsoft Office (Excel, Teams, Outlook).
  • Experience with Electronic Records software is preferred.
  • Strong communication and organizational skills are essential.

Responsibilities

  • Coordinate ISP development, updates, and meetings for assigned people supported.
  • Monitor progress toward ISP goals and coordinate with CSS, families, and teams.
  • Prepare intake paperwork, track service authorizations, and support transitions.
  • Maintain accurate documentation and timely reports for person-centered supports.
  • Communicate professionally with people supported, families, and partners to ensure dignity and continuity of services.
  • Provide on-the-floor support as needed, including assistance with ADLs/IADLs.

In this role, you will support individuals participating in Day Support Activities (DSA) by coordinating Individual Support Plans (ISPs), maintaining accurate documentation, supporting intake and transition processes, and serving as a key communication link among people supported, families, home providers, case managers, Services Coordinators/Personal Agents (SC/PAs), Program Managers, Community Support Specialists, and other support team members.What You'll DoCoordinate ISP development, updates, packets, meetings, training, documentation, and follow-up for assigned people supported.Monitor progress toward ISP goals and partner with CSS staff, Program Managers, families, home providers, case managers, SC/PAs, and other team members to address changing support needs.Review referral packets, coordinate intake paperwork, track service authorizations, and support exits or transitions with clear communication and timely documentation.Coordinate updates related to medical, behavioral, safety, attendance, adaptive equipment, or other person-specific supports, including staff training documentation when needed.Communicate professionally and empathetically with people supported, families, advocates, internal teams, and external partners to support dignity, safety, inclusion, and continuity of services.Provide direct “on the floor” support as needed, including assistance with ADLs and IADLs, and prepare clear, timely reports and related documentation.Why KerrIf you are organized, collaborative, and motivated by person-centered human services work, we would love to hear from you!Paid medical, dental, and vision employee benefitsPaid basic life & accident insurancePaid internal training and career developmentPaid holidays and generous time off401(k) retirement savings plan with up to 4% employer match24-hour Employee Assistance Plan (EAP) that provides resources for everything from mental health to pet insurance and financial counsellingBachelor's degree in a related field and at least 2 years of case management experience OR 4 years of relevant experience with foundational knowledge of social services.Proficiency with software applications and tracking systems, including Microsoft Office (Excel, Teams, Outlook).
