Job summary

YAI is currently seeking a Full Time Individual Service Coordinator (ISC) to join our Day Programs in Florida. In this role, you will manage a caseload of individuals, advocating for their needs and coordinating between programs, families, and care managers.

The ideal candidate has a BA and experience with intellectual and developmental disabilities. Exceptional interpersonal and organizational skills are required, as well as a familiarity with Evero. This is a fantastic opportunity to make a significant impact in the lives of those we support.

Qualifications

• Experience working with people with intellectual and developmental disabilities.
• Ability to advocate for individuals' needs and desires.

Responsibilities

• Manage a caseload of 50-70 PWS.
• Input services and meeting minutes into Evero.
• Communicate with challenging families and residences.
• Maintain records in accordance with OPWDD regulations.
• Provide coverage based on program needs.
