Job summary

YAI is seeking a Full Time Individual Service Coordinator (ISC) in Baton Rouge, Louisiana. The ISC will provide case coordination and advocate for individuals between programs, families, residences, and care managers.

The ideal candidate will manage a caseload of 50-70 individuals, input service data, and maintain compliance with regulations. A BA is preferred, with experience in supporting individuals with disabilities.

The compensation is $19.05 per hour, and YAI promotes an equitable workforce environment.

Qualifications

• BA preferred.
• Experience working with people with intellectual and developmental disabilities.
• Advocating for the needs/desires of individuals.

Responsibilities

• Manage a caseload of 50-70 PWS.
• Input services and meeting minutes into Evero.
• Communicate with challenging families/residences.
• Maintain records according to OPWDD regulations.
• Assist with audits.
