Job summary

Life's WORC is seeking staff for Day Habilitation programs in New York. The role involves overseeing participants, implementing individualized plans, and providing training and support to ensure safety and progress.

The position requires a high school diploma, experience with individuals with developmental disabilities or autism, and a valid NY driver’s license. Travel to sites across Queens, Nassau, and Suffolk may be necessary.

Qualifications

• High School Diploma or equivalent.
• Prior experience assisting persons with Intellectual/Developmental Disabilities and/or Autism.
• Ability to safely drive an Agency van for a majority of the day.
• Excellent verbal and written communications skills with the ability to document services electronically.
• Ability to work independently.
• Valid NY State Driver’s License with less than 8 points required.

Responsibilities

• Provide ongoing oversight to people attending the Day Habilitation Program.
• Implement the Day Habilitation plan for each person following stated safeguards.
• Provide training and support for each person in accordance with the individual personalized program plan.
• Implement behavioral support plans as written and to ensure associated data collection.
• Train in and demonstrate competency of CPR, SCIP, Person-Centered Planning and Defensive Driving.
• Drive agency vehicles and demonstrate safe driving practices.
• Travel train program participants via public transportation.
• Establish and maintain a rapport with community supports.
• Flexibility to work at varied Day Habilitation sites located in Queens, Nassau and Suffolk Counties if needed.

• The pay rate range for this position is $20.00-$21.49 per hour depending on years of prior experience with NYS OPWDD programs
• Must have valid Driver's License
