Job summary

Life's WORC Trust Services - Official Page is seeking a Day Habilitation Supervisor to oversee participants in our Day Habilitation Program. The role involves implementing individualized plans, training staff, and ensuring safety protocols are followed across multiple sites in Queens, Nassau, and Suffolk Counties.

The candidate must have a High School Diploma or equivalent, a NYS Driver's License with limited points, and prior experience with IDD/Autism.

Qualifications

• High School Diploma or equivalent.
• Prior experience assisting persons with IDD and/or Autism.
• Ability to safely drive an Agency van for a majority of the day.
• Excellent verbal and written communications skills with the ability to document services electronically.
• Ability to work independently.
• Valid NY State Driver’s License with less than 8 points required.

Responsibilities

• Provide ongoing oversight to people attending the Day Habilitation Program.
• Implement the Day Habilitation plan for each person following stated safeguards.
• Provide training and support for each person in accordance with the individual personalized program plan.
• Implement behavioral support plans as written and to ensure associated data collection.
• Train in and demonstrate competency of CPR, SCIP, Person-Centered Planning and Defensive Driving.
• Drive agency vehicles and demonstrate safe driving practices.
• Travel train program participants via public transportation.
• Establish and maintain a rapport with community supports.
• Flexibility to work at varied Day Habilitation sites located in Queens, Nassau and Suffolk Counties if needed.
