Job summary

Life's WORC is seeking a Day Hab Supervisor to oversee participants in the Day Hab program at multiple sites in Queens, Nassau, and Suffolk Counties. The role involves implementing individualized plans, supervising daily activities, and ensuring safety during transportation via agency vehicles.

Qualifications

• High school diploma or equivalent required.
• Prior experience assisting persons with Intellectual/Developmental Disabilities and/or Autism.
• Valid NY State Driver’s License with less than 8 points required.
• Ability to drive an Agency van for most of the day.

Responsibilities

• Provide ongoing oversight to people attending the Day Habilitation Program.
• Implement the Day Habilitation plan for each person following stated safeguards.
• Provide training and support for each person in accordance with the individual personalized program plan.
• Implement behavioral support plans and ensure data collection.
