Responsibilities

• Collaborate with individuals to identify interests, strengths, and goals and develop meaningful daily schedules.
• Provide direct support and coaching to help individuals complete tasks and achieve goals.
• Follow individualized behavioral care plans and encourage participation and progress.
• Maintain accurate documentation of progress and outcomes for continuity of care.
• Provide a safe, supportive environment and advocate for independence and choice.
