Responsibilities

• Monitor program implementation and participant outcomes with regular assessments.
• Maintain documentation of daily services and progress in health systems.
• Implement individualized life plans and promote independence and social skills.
• Communicate with the Day Habilitation Manager and teams to track goals.
• Provide safe transportation and maintain vehicle logs.
