Responsibilities

• Person-Centered Community Engagement: identify interests and goals, develop schedules including community activities.
• Goal-Based Skill Development: teach tasks and coach independence.
• Behavioral Support Implementation: apply approved strategies, encourage participation.
• Documentation & Progress Tracking: maintain progress records for continuity of care.
• Safety & Advocacy: provide a safe environment and advocate for independence.
