Job summary

Vinfen seeks a Day Hab Program Activity Assistant (Instructor) to aid in implementing clinical and individual programs within the Day Hab setting. The role includes developing DHSP plans, supporting persons served, and collaborating with the interdisciplinary team to ensure meaningful daily activities and progress.

Responsibilities include instruction in self-help and social development, maintaining documentation, and contributing to crisis management with PBS and Safety Care.

Qualifications

• One year of relevant work experience/training.
• High school diploma or equivalent required.

Responsibilities

• Provide instruction and skill-building in Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development.
• Assists in case management responsibilities as directed.
• Develop and implement DHSP for persons served, including recommendations from professionals and staff.
• Maintain daily documentation and progress notes per guidelines.
• Engage persons served individually and in groups, developing daily living skills.
• Collaborate with management and interdisciplinary teams to inform treatment and programs.
