Job summary

Paycom in Santa Barbara, CA is hiring a D-SNP Care Coordinator to support non-licensed care management services for eligible members. You will lead member outreach, enrollment, and follow-up, working closely with RN Care Managers and Social Workers to promote access to needed services.

Qualifications

• Associate’s degree in business, health field, or related area, or 2–3 years in a medical setting.
• 2–3 years of full-time or equivalent experience in medical offices with patients.
• Experience with vulnerable populations is preferred.

Responsibilities

• Conduct outreach for enrollment and consent into Care Management programs.
• Coordinate appointments, transportation, and referrals with the care team.
• Maintain accurate logs and timely documentation in care management systems.
• Participate in ICT meetings and escalate non-clinical concerns per protocol.
