Job summary

Qualifications

• Current and unrestricted RN license in CA with at least 3 years of experience in a nursing/managed care setting.
• Certification in Case Management (CCM/ACM) or obtain within two years of employment.
• BSN preferred; bilingual abilities beneficial.

Responsibilities

• Conduct comprehensive member assessments of medical, behavioral, functional, and psychosocial needs.
• Develop individualized care plans and monitor progress toward goals.
• Coordinate interdisciplinary care with PCPs, specialists, and community resources.
• Educate members and caregivers on benefits, resources, and self-management.
