Job summary

The Texas Department of State Health Services seeks a CYSHCN Health Coordinator to lead the Title V Block Grant program for Children and Youth with Special Health Care Needs. This senior role plans, develops, and implements care systems and collaborates with agency programs and external partners to improve public health outcomes.

Under supervision of the CYSHCN Systems Development Group Manager, you will oversee data analysis, policy development, and stakeholder engagement, ensuring alignment

Qualifications

• Public health principles and quality improvement methods.
• Knowledge of national and state CYSHCN performance measures.
• Experience with policy development and grant monitoring.
• Proficient with MS Office tools for reports.

Responsibilities

• Attends work regularly and follows agency leave policy.
• Leads CYSHCN State Action Plan development and monitoring (35%).
• Drives program quality improvements and staff development (20%).
• Develops and disseminates CYSHCN education materials and trainings (15%).
• Builds partnerships with state agencies and community groups (10%).
• Represents the program at meetings, conferences, and boards (10%).
• Reviews legislation relevant to CYSHCN and coordinates grants (5%).
• Performs other duties including disaster response and COOP as needed (5%).
