Job summary

CareAparent is seeking a full-time Certified Wound, Ostomy, and Continence Nurse to join our Clinical Team. You will perform wound assessments, develop individualized care plans for complex wounds, and coordinate on-site LPN and caregiver visits to ensure adherence to the plan of care.

You will also conduct home health assessments, manage ostomy care and education, document findings in CareAparent EMR, and collaborate with therapists and physicians to optimize client outcomes while maintaining

Qualifications

• Must possess Minnesota RN license and CWOCN certification.
• BSN preferred; hospital or home health experience a plus.
• Minimum 1 year of home care or assisted living experience.

Responsibilities

• Wound assessment and management per plan of care.
• Coordinate on-site LPN and caregiver visits and OASIS-like assessments.
• Document assessments and visits using CareAparent EMR.
• Educate clients and families on ostomy and wound care.
• Collaborate with interdisciplinary team to update care plans.
