Qualifications

• Active Minnesota RN license required.
• CWOCN certification required.
• Experience in wound, ostomy, continence care preferred.
• At least 1 year of home care or assisted living experience preferred.

Responsibilities

• Perform thorough wound assessments and manage complex wounds, creating care plans.
• Provide ostomy care education and appliance management.
• Coordinate with on-site LPNs and caregivers to ensure Plan of Care adherence.
• Conduct OASIS assessments and document patient visits in EMR.
