Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.
• Experience in the clinical area for which applying is desired.
• Bachelor's degree preferred.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates a plan of care with discharge planning and multidisciplinary input.
• Implements the plan of care and coordinates nursing activities with the health care team.
• Evaluates patient progress and informs care plan updates.
• Documents assessment, plan, implementation and evaluation in the patient record.
