Qualifications

• RN license in New York state and current nursing license.
• Graduation from an accredited school of nursing.
• BSN degree preferred per NYS requirements.
• Specialty credentialing and ongoing education.
• Maintains licensure and certifications as required.

Responsibilities

• Collaborate and facilitate delivery of care via digital platforms.
• Assess, plan, implement, coordinate and evaluate patient care from admission to discharge.
• Apply critical thinking and case management principles to care plans.
• Monitor quality metrics and document accurately in records.
• Ensure care meets regulatory standards and best practices.
• Keep knowledge updated on healthcare economics and reimbursements.
