Job summary

Family Service League Inc. is seeking a full-time Peer Specialist in Mastic, NY to provide care management from a lived-experience perspective for adolescents and adults with behavioral health needs.

The role involves field work and hospital-to-community transitions, within an interdisciplinary team, for about 9 months post-discharge. We offer comprehensive benefits including health, dental, vision, FSA, retirement with employer contribution, life/AD&D, generous PTO, holidays, EAP, disability

Qualifications

• High school diploma or equivalent required.
• Certified Peer Specialist or Youth Advocate required.
• Relevant life experience required and ability to draw on it as a role model.
• Prior experience in a behavioral health setting preferred.
• Excellent interpersonal and verbal and written communication skills.
• Ability to solve problems, make clear decisions and work independently or in a team.
• Proficient computer skills including Microsoft Office and EHRs.
• Bilingual in Spanish strongly preferred.
• Valid and clean NYS Driver's License required.

Responsibilities

• Provide care management from a lived-experience perspective to adolescents and adults with behavioral health needs.
• Collaborate on referrals and interventions within a multidisciplinary team.
• Coordinate care planning with providers and ensure goal-directed care.
• Assist with community linkages and resources to support transitions.
• Complete clinical documentation in accordance with NYS OMH in an electronic system.
• Attend supervision meetings and maintain de-escalation proficiency.
