Job summary

The Bridge is seeking an Assistant Director to lead a CTI team in delivering comprehensive, community-based support to adults with serious mental illness and co-occurring substance use disorders. You will supervise staff, coordinate care, and engage with hospitals, DHS, and families to ensure seamless transitions and resource connections.

Qualifications

• Licensed master’s degree or higher in Social Work, Mental Health Counseling, Nursing, or Psychology.
• Experience working with individuals with serious mental illness (SMI) and co-occurring substance use disorders (SUD).
• Knowledge of community resources and counseling principles for mental illness and substance use disorders.
• Ability to develop, evaluate, implement, and modify treatment interventions to meet needs.
• Ability to prepare accurate and timely reports.
• Computer proficiency in health IT and Microsoft Office applications.

Responsibilities

• Provide clinical and operational leadership to the CTI team in collaboration with the Program Director.
• Conduct outreach and engagement using strength-based approaches, starting during inpatient admissions or ED visits.
• Coordinate with hospitals, DHS, and family members to identify individuals in need of outreach and care.
• Assess health and social needs using CTI methods and formal risk assessment tools.
• Participate in discharge planning to connect participants with community resources.
• Provide short-term therapeutic counseling pre- and post-discharge.
• Collect and report data to inform care delivery with the team.
