Qualifications

• Bachelor’s degree in nursing required.

• Current BLS certification.

Responsibilities

• Coordinate complex ambulatory appointment scheduling and records maintenance; triage and follow-up communication with referring providers and patients.
• Coordinate clinical care management including discharge planning, pulmonary rehab, home care, hospice, equipment and therapy referrals; triage calls.
• Manage authorizations and payer communications to secure timely approvals for procedures and therapies.
