Kentucky: KBMIRT Radiation Operator's License - Radiography (Temporary or Permanent). Associate has 12 months from issuance to obtain ARRT(R).
Indiana: Indiana State Department of Health License (Radiologic Technologist or Student Permit). Associate has 6 months from issuance to obtain ARRT(R).
