Job summary

LiveWell Homecare Agency, LLC seeks an RN to provide patient- and family-centered care in a Lansdale location. You will apply the nursing process to address physical, psychological, emotional, and social needs and develop individualized care plans with patients and families.

Qualifications

• Accredited nursing diploma and active RN licensure in the practice state.
• BSN preferred within three years.
• Strong communication, assessment, education and documentation skills.
• BLS certification required.

This nursing role focuses on delivering patient- and family-centered care by applying the nursing process to address the physical, psychological, emotional, and social needs of diverse patients.

• Develops individualized care plans in collaboration with patients and care partners, respecting cultural beliefs and values.
• Engages in effective communication with patients, families, and healthcare teams, incorporating evidence-based practice and cultural awareness.
• Provides education tailored to patient and family needs, minimizing risks and supporting health literacy.
• Utilizes health information technology for communication, documentation, and quality improvement efforts.
• Collaborates within inter-professional teams, fostering respect, open communication, and shared decision-making.
• Contributes to creating an inclusive environment, participates in safety and quality initiatives, and seeks ongoing professional development.
• Qualifications include an accredited nursing diploma, with a preferred BSN within three years, and active RN licensure in the practice state. Prior acute care experience is preferred. Must hold BLS certification and demonstrate strong communication, assessment, education, and documentation skills.
