Job summary

Northern Rivers Family of Services in New York seeks a Community Service Provider to deliver home and community-based services for children with emotional and mental health challenges.

You will travel locally and work onsite in communities, coordinating with families, documenting plans, and collaborating with supervisory staff to ensure effective care and outcomes.

Qualifications

• Bachelor’s degree and at least two years of related experience OR Master’s degree with at least one year of related experience.
• Valid NYS Driver’s License required.

Responsibilities

• Provide services within a Family-driven Care framework, respecting family choice and voice.
• Work with family members to identify goals and objectives of treatment.
• Coordinate with the child and caregiver on scheduling and provision of services.
• Participate in treatment meetings and trainings; complete all documentation.

As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite.
