Northern Rivers Family of Services is seeking a Community Service Provider to deliver home and community-based services to children with emotional and mental health challenges. You will travel locally to engage families in their communities, providing rehabilitative services, non-licensed counseling, skill-building, caregiver support, and advocacy.

• Bachelor's degree in a related field and at least two years of related experience OR Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
• Valid NYS Driver's License

• Work with family members to identify goals and objectives of treatment
• Coordinate with child and caregiver on scheduling and provision of services
• Attend relevant treatment meetings and required trainings; complete documentation

As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite.
